Observer bias in RCTs

Non-blinded observers in clinical trials are biased. This paper in the BMJ explores the effect for certain outcome assessments where a binary classification of patients was required. What does this say for ongoing workplace evaluations where clinicians are not routinely 'blinded'? It highlights the importance of trying to remove bias when classifying a patient outcome.
